" The new Onsite Monitoring Waiver request form is now available for eligible CACFP sponsors of child care centers, day care home providers, and adult day care centers. Through September 30, 2025, sponsors with an approved waiver can conduct one annual monitoring review offsite and conduct a second offsite monitoring review for those centers and daycare homes that are more than 100 miles or two hours travel time away from any sponsoring organization’s offices. "
The new Onsite Monitoring Waiver Request Form is now available for eligible CACFP sponsors of child care centers, day care home providers, and adult day care centers. Through September 30, 2025, sponsors with an approved waiver can:
• Conduct one annual monitoring review offsite; and
• Conduct a second offsite monitoring review for those centers and daycare homes that are more than 100 miles or two hours travel time away from any sponsoring organization’s offices.
Restrictions for this waiver include:
• Sponsors must conduct at least one unannounced onsite monitoring review of each site/provider, including observation of meal service, annually.
• Sponsors that utilize review averaging cannot utilize this process for sites/centers that will only be visited twice a year, regardless of distance from the sponsor’s office.
• All pre-approval visits, first four-week reviews for new sites/providers, and Serious Deficiency follow-up reviews must be conducted in-person.
• Timing of reviews must adhere to federal requirements as described in the CACFP Handbooks.
• Sites monitored remotely must be in good standing.
• To comply with USDA requirements, approved sponsors must provide information about offsite monitoring implementation to TDA upon request.
To apply for this waiver, CACFP sponsors must:
• Download and complete all questions in the CACFP Sponsor Offsite Monitoring Implementation Plan. This form is also available on the CACFP Administration/Forms page of SquareMeals.org.
• Click on the CACFP Sponsor Onsite Monitoring Waiver Request Form, complete all required questions, and upload the completed CACFP Sponsor Offsite Monitoring Implementation Plan before submitting.
Once your waiver request and implementation plan have been reviewed, TDA will notify you of its decision. If approved, this waiver may be used through September 30, 2025. Note that TDA may reach out to you for more information prior to approving this waiver.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
