Financial Report

2025 Annual NSLP Financial Report (FY2024 Data)

This report must be completed by School Funded Authorities (SFAs) who participate in the NSLP, School Breakfast Programs (SBP), Special Milk Program (SMP) or At-Risk CACFP programs and must be completed by an authorized representative. The SFA must ensure that the reported data is accurate and reflects the SFA’s financial operations for the fiscal period reported.
General Information
  • The Annual NSLP Financial Report will remain in JotForm. 
  • SFAs will receive an email notification indicating the JotForm is available. 
  • Data used for the 2025 SY will be the prior fiscal year (2023-2024) from audited financial reports. For public school districts and charter schools, this is the same data submitted to TEA.
  • The Financial Report must be completed by an authorized representative. The SFA must ensure that the reported data is accurate and reflects the SFA’s financial operations for the fiscal period reported.
  • If data is determined to be invalid corrective action will be required.
Required Due Date
  • The 2025 NSLP Financial Report submittal due date deadline is to be determined.

The Preparing for the Financial Report form has been provided to prepare the SFA in completion of the NSLP Financial Report. It contains a list of documents, terminology, object codes, and additional pertinent information related to the NSLP Financial Report.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
