"
TDA has identified compliance issues arising at unaffiliated At-Risk sites managed by sub-sponsors that themselves are not affiliated with the location of service or the enrichment being provided at the location. Some of these sub-sponsors are signing site agreements with Contracting Entities to provide the meal service only (a circumvention of vended meal vendor procurement processes) and/or are taking over administrative tasks that should be performed by the primary sponsor.
Beginning February 16 and no later than June 30, all sponsors of unaffiliated At-Risk Sites are required to submit a questionnaire/attestation form confirming that they are conducting all required oversight functions at their unaffiliated sites and that those sites are responsible for the oversight of a complete afterschool enrichment program.
For any site that TDA has not received an attestation form by June 30, 2024, it will begin closing those sites in TX-UNPS with an effective closure date of September 30, 2024. All sponsors of unaffiliated At-Risk sites must complete this form on behalf of all allowable sites.
TDA will be holding a webinar for At-Risk sponsors on February 21. Please continue to monitor SquareMeals.org for news, FAQs, and the webinar registration link. TDA will also begin sending out bi-weekly reminders to all applicable sponsors beginning in mid-February.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
