It is that time of the year to submit renewals and the new agreements for the Farmers’ Market Nutrition Program (FMNP) and The Senior’s Farmer’s Market Program (SFMNP) for Program Year 2024-2025 in TX_UNPS. It is that time of the year to submit renewals and the new agreements for the Farmers’ Market Nutrition Program (FMNP) and The Senior’s Farmer’s Market Program (SFMNP), CFDA #10.572 for Program Year 2024-2025 in TX_UNPS.
Please note that the new PY 2025-2027 FMNP Agreements will be completed this year, as all older agreements will expire on December 31, 2024.

o Form H1420- Farmers Market Location and Site Information
o Form FMNP-01-FMNP Agreement Between TDA and Contracting Entity- revised 9/2024
o Form FMNP-01A -FMNP Agreement Between TDA and Contracting Entity – AMENDMENT- revised 9/2024 (If the CE fully operates and controls the FMA, must also complete and submit)
o The following agreements will not be signed by or submitted to TDA. These agreements will stay at the CE level and reviewed during Administrative Reviews. Please complete the following and keep your records.
- Form FMNP-02-FMNP Agreement Between Contracting Entity and Famers’ Market Association- revised 9/2024

Note: To submit the application to TDA for review, ensure that you have checked the box next to each document, “submitted to TDA”, even if it doesn't apply and was not uploaded, as you will not be able to submit the application for approval overall. Then click the red submit for approval button.
NOTE: The Checklist documents can be uploaded to your packet by clicking the Upload Checklist Item Documents hyperlink and then clicking Add Attachment.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
