Summer operators approved to serve non-congregate meals in rural areas must still adhere to all SFSP or SSO regulations, including monitoring, meal counting and claiming, and participant notification requirements. TDA has released the following new prototype forms to assist summer operators in documenting operational activities related to non-congregate service:
• SFSP Pre-Operational Visit Form
• SFSP First Two Weeks Site Visit Form
• SFSP Site Review Form for Non-Congregate and Hybrid Sites
• Updated SSO Onsite Review Form (with new questions pertaining to non-congregate service)
• Meal Count Forms for Sites Bundling Non-Congregate Meals
• Meal Count Form for Standard Congregate Service
• Participant Notification Form for school sites approved to operate a Conditional Non-Congregate Site
Additionally, Summer Non-Congregate FAQs have been updated with new clarifying guidance. Please visit the Summer Non-Congregate Feeding Program page on SquareMeals to access the forms and the FAQs. The forms are also located on the NSLP and Summer Meals Administration/Forms pages, as applicable.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation.
