School Meal Application Letters
Please Note
TDA has revised the sample letters. The description provided for each letter explains the appropriate use of each letter.
For School Year 2022-2023, TDA has updated the sample letters to households to include the new nondiscrimination statement. However, to assist school districts in communicating the transition from free meal distribution to all students in SY 2021-2022 back to the standard free/reduced-price/paid model under NSLP, TDA recommends that districts use brand new resources located on the brand new Back to Student Meal Rates page. These resources can be emailed, sent home with students, or mailed with the applications.

Media Release Prototype
NOTE: In addition to the standard media release prototypes listed below, TDA has prepared a new media release template for districts that operated SSO last year and want to communicate the transition back to the standard paid/reduced/free NSLP model. Regulations require that prior to or during the first week of school, a public release be made to the local news media, the unemployment office and any major employers who are contemplating layoffs within the school’s attendance area. When sent to the news media, the letter to parents, application form, and a complete income scale (free and reduced-price guidelines) or the same information put in a narrative form will constitute a public release. TDA has provided this 
prototype media release (Spanish version) or media release Provision 2 (Spanish version) and media release CEP (Spanish version) for CEs to customize and use each school year.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-Complaint-Form-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
