Requesting MENU Module Access for CE and ESC Users
Access to the MENU Module can then requested by either the Educational Service Centers (ESC) and Contracting Entities (CE) in order to start using the module. Review the following MENU Module Access Tips and use the steps below to request access through TXUNPS:
Step 1
Plan the date for the required access to MENU Module for Training and/or Production.
Step 2
Once the date has been determined:
ESC Users:
- Submit a Certificate of Authority for Internal Users (FND-100) form at least 3 weeks prior to the planned date to require access to allow time for processing. Please refer to the TDA ESC SharePoint site for detailed instructions to complete and submit the form to access TXUNPS.
- For MENU access via TXUNPS, check the MENU CE Support Add box on the FND-100 form in Other Groups in Section A of the form.
 
CE Users:
- Submit a Certificate of Authority for External Users (FND-101) form at least 3 weeks prior to the planned date to require access to allow time for processing.
- Complete Sections A, B and D of the form per the instructions on the form.
- For MENU access via TXUNPS, check the MENU CE Support Add box on the FND-101 form in School Nutrition Program (SNP) Groups in Section B of the form.

Step 3
Either FAX or mail the form based on the instructions at the bottom of the form.

Users will receive notification of access granted, User ID, Password instructions and a link to TXUNPS via the email address provided on the form.
Please NOTE: A user must login at a minimum of every 90 days or the user will be deactivated requiring a new form to be submitted to regain access.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
