NEW At-Risk sites that do not qualify for area eligibility (50% Free and reduced)
  1. If a new At-Risk site does qualify using area eligibility (50% Free and reduced) then complete the site application as per normal requirements.
  2. If a new At-Risk site does NOT qualify using area eligibility, then complete the site application according to the guidance below:
    1. Question A11: Enter information as requested.
    2. Question A12:
      1. Select web address.
      2. Enter USDA Waiver #93 in the text box.
Complete the rest of the site application as per normal requirements. Submit health and safety, enrichment, and letter of exemption of license information.

NOTE: TDA will conduct proximity checks for all NEW At-Risk sites and may request additional documentation from the CE.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
