TDA has updated the Child Care Center Meal Production Record (H1530) to reflect the expanded milk choice options for children ages 2 and up as described in Final Rule - Expanding Fluid Milk Options in Child Nutrition Programs.
Given that whole milk can be served to children 2 and up in addition to 1-year-olds, who must receive unflavored whole milk, TDA has modified the form so that sites can now account separately for whole milk prepared just for 1-year-olds and whole milk prepared for participants ages 2 and up. Sites can now specifically list the amount of milk prepared by type, which will allow sponsors to better assess milk actually purchased and prepared for each meal.
Sponsors and sites that use alternate versions of the meal production records should begin updating their own forms/systems as soon as practicable if planning to expand milk options beginning June 8.
The updated H1530 form is located on the CACFP Administration/Forms page of SquareMeals.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
