" The H4529 Transfer Form provides a consistent process for managing USDA Foods transfers and helps ensure that inventory is properly accounted for throughout the process. It documents the organizations involved in the transfer, the USDA Foods being transferred, the quantity requested, and the approval needed before the transfer takes place.

TDA HAS LAUNCHED THE H4529 TRANSFER FORM FOR PROCESSING AND DIRECT DELIVERY
The request will be reviewed once a week.
Before submitting, please see the requirements below.
Releasing Inventory requirements
- No school-to-school transfers are permitted.
- If you release inventory, the ESC will be contacted to provide technical assistance for utilization.
- If inventory is not used, a courtesy copy will be sent to your Superintendent indicating that the inventory has been forfeited.
- Review your current inventory and confirm that the months on hand are one month or less.
- Monthly usage must be 10%
Requesting Inventory requirements
- Request will depend on inventory availability
- If there is no inventory available, the request will be put on a waiting list

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
