Preliminary Verification Report Due November 15!

October 27, 2021 - The Preliminary Verification Report will open November 1 and is due November 15. The report is a JotForm available through Download Forms in TX-UNPS (SNP-123). All CEs are required to complete the report. However, schools operating SSO, CEP, or P2 will complete a limited number of fields.

The School Food Authority (SFA) Verification Report is due in TX-UNPS no later than February 1. The count of students reported on the Preliminary Verification Report MUST equal the count of students reported on the SFA Verification Report. Even though the data is reported differently in the two reports, it is the same data.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: How to File a Complaint, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
