Local Wellness Policy Flexibility for School Year 2022-2023 – Request Form is LIVE

" July 1, 2022 - TDA was given the discretion to allow NSLP/SBP CEs to complete their first triennial Local Wellness Policy assessment by June 30, 2023. CEs planning to use the new deadline must inform TDA by July 15, 2022, via request form available on the Intake Forms page of SquareMeals.org.

July 1, 2022 -  USDA issued a waiver allowing CEs to complete their first triennial Local Wellness Policy assessment by June 30, 2023. CEs electing to use new deadline must complete the second triennial assessment by June 30, 2026. For operators that completed their triennial assessments by the original extended date of June 30, 2022, second assessments will be due June 20, 2025.

CEs electing to use the new deadline must notify TDA by July 15, 2022 by completing the Notification to Extend First Local Wellness Policy (LWP) Triennial Assessment form available on the
Intake Forms page of SquareMeals.org click here.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-Complaint-Form-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
