A Summer Food Service Program (SFSP) Sponsor is defined as sponsors currently participating in SFSP applying for the at-risk afterschool care center component only.
SFSP sponsors that are currently in good standing (meaning they are not currently in the serious deficiency process) have a streamlined application process when applying to participate in the at-risk afterschool care center component of the CACFP. However, TDA may request additional information for any applicant during the application process as needed.
NOTE: SFSP sponsors that experienced significant operational difficulties will be required to submit additional information, as requested by TDA.
The information contained in the bulleted list below must be completed and/or submitted.
- CE application screen in TX-UNPS
- Site application in TX-UNPS for each at-risk site
- Board of Directors screen in TX-UNPS
- Governing Body Awareness form
- Legal Documents(applies to all Principals within non-governmental organizations)
- Copy of Government Issued ID
- Driver’s license
- Military ID
- Valid US Passport
- Voter Registration Card
- Proof of residential mailing address
- Driver’s license
- Copy of a current lease
- Bank statement
- Utility bill
- Performance Bond, if applicable
- Budget Detail screen in TX-UNPS
- Checklist items –
- Documentation of license or exemption (if applicable)
- Documentation of compliance with local health and/or safety requirements (inspection report), if not provided during the sites’ SFSP participation
- Description of qualifying afterschool program that includes educational/enrichment activities
- Documentation from the qualifying school verifying the site’s location in the school’s attendance zone (if census data was used to qualify the site in SFSP)
- Permanent Agreement Contracting Entity Specific Amendment
- The Permanent Agreement must be amended to add participation in the CACFP.
All forms that are not submitted in TX-UNPS must be completed in ink or other non-erasable print.
