- Question 5. Site address: Enter the physical site address where meals are served. Please ensure that the address can be verified via the USPS Zip Code look-up, this will ensure that it will be mappable on the TDA summer meal mapper.
- Question 9. Year round school: Should only be answered yes, if this site is categorized with TEA as year-round. If serving just in the summer, this should be answered, no.
- Question 10. Operation Dates: All School Food Authorities that are mandated to operate SSO, are required to operate for a minimum total of 30 days. Please ensure the data range is 30 days, if mandated.
- Question 13. Site type: Restricted Open & Closed Enrolled: The USDA waivers have ended, the regular guidance for these site types must be followed. Please ensure that the explanation entered follows the guidelines. For more information on the site type please refer to the ARM section 11: Summer Meals.
- Question 13. Qualified By: Choose either the first bubble next to Free and Reduced approved school date and in the drop down menu choose the site to qualify or use census data.
*Once the SSO site applications have been completed, please ensure that the application overall is submitted, by clicking the red submit for approval button.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-Complaint-Form-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
